Set Up Package Preparation

I have completed your shipper and carrier set up packages as far as I can go. Please review these pages very carefully and if there are minor mistakes, you may be able to take care of them yourself. Otherwise, let me know what needs to be changed and I’ll make the changes.

There are several pages that you will need to complete yourself as follows:

For the shipper set up package:

* Letter of Introduction – complete this as you see fit – keep it simple

* Grant Letter – replace this with your’s

* W-9 – in Session 3.1, download the form, sign it, date it, scan it and put it into your set up package

* BMC 84/85 – replace this with your’s

For the carrier set up package:

* Letter of Introduction – complete this as you see fit – keep it simple

* Grant Letter – replace this with your’s

* Billing Info – modify as you see fit

* BMC 84/85 – replace this with your’s
